Implications for Children Diagnosed at Clinical Onset of T1D Without Early Identification
Rosa is an 8-year-old child with presymptomatic autoimmune T1D, but neither she nor her family know this yet. Without prior knowledge, she may be at increased risk of DKA at clinical onset of T1D
Learning Objectives
- Recognize the benefits of prior knowledge of at-risk status before clinical onset of autoimmune T1D in children

Summary
Rosa is an 8-year-old child with presymptomatic autoimmune T1D, but neither she nor her family know this yet. Let’s look at the potential implications if Rosa’s parents opt not to screen for T1D, and she develops Stage 3 diabetes without early identification. She will experience progressive destruction of beta cells, a reduction in endogenous insulin, and, finally, insulin deficiency. And there is a big risk of DKA, which can cause brain changes and neurocognitive defects. There is a lot of data demonstrating that increased morbidity and mortality in people with type 1 diabetes is largely due to DKA. So DKA should be taken very seriously.
